Errors use HTTP status codes with a Problem Details body. Typical members include type, title, status, detail, and a machine-readable code. Common statuses: 400, 401, 403, 404, 409, 422, 429, 500, 503. OTP-specific codes such as otp_challenge_expired and otp_challenge_locked are defined in the OpenAPI contract.
